To all contractors category GC/BC 7 and above with license valid for the year 1998 E.C. Renewed Registration Certificate, VAT registered certificate, clearance given by Oromia Finance Economic Development Bureau witch is showing that the bidders has paid all required government taxes and bidders should also submit experience letters.

1. Bale Zone Health office invites wax-sealed bids from finically and technically capable bidders to furnish the necessary materials, labour and equipment for construction and completion of Numbers Health center at the following locations:

I. Dinsho Nucleus Health Center in Sinana-Dinsho Woreda

II. Serofta Uncles Health Center in Dodola Woreda

III. Dollo-Sebro Nucleus Health Center in Ginir Woreda

IV. Ali Nucleus Health Center in Agarfa Woreda

V. Oborra Nucleus Health Center in Sinana-Dinsho Woreda

VI. Dawe Sarar Uncles Health Center in Dawe-Sarar Woreda

2. Bidding document and additional set of document may be purchased on submission of written application to Bale zone Health Office for non-refundable fee of Birr 100 (One Hundred Birr) working days and hours.

3. Bidders are requested to fill all necessary details in bidding documents and are also instructed to fill, sign and seal each page of bid document. Failure to follow this instruction any cause automatic rejection.

4. The bid document shall be filled and completed in two copies and returned back in separate wax-sealed envelopes labeled ?Original? and ?Copy?. Copy must be ?machine photo copy?

5. All bids must be accompanied by bid security of at least 1% of bid amount in acceptable CPO or Bank Guarantee valid for 90 calendar days and smut be delivered to Employer on or before 3^rd day of May 2006 (Miazia 25/1998 EC) 12:00 o?clock (noon).

6. Bids will be opened in presence of bidders or their authorized representatives who choose to attend in the same date at Bale Zone Administration Office.

7. The construction works shall be completed within a maximum of 270 calendar days from commencement of works.

8. The Employer reserves the right to reject any or all bids.
