SUB-LOT IE-Miscellaneous

SUB-LOT IF-Drug for opportunistic infection

LOT-II: Medical supplies

LOT-III: Laboratory reagents

LOT-IV: Medical Equipment

Sub Lot A Medical Equipment for Health Center

Sub Lot B Medical Equipment for Health Post

2. Interested eligible bidders may obtain further information from and inspect the bidding documents at the address given below: Office of Planning and programming Department, Ministry of Health, P.O. Box 1234, Woreda 3, Kebele 53, Tel. 15 20 54, Fax 51 26 91, Addis Ababa, Ethiopia from 8:30-12:30 A.M. and 1:30-5:30 P.M. at office hours.

3. A complete set of bidding documents may be purchased by interested bidders on the submission of a written application to the address below:

Office of Planning and programming Department, Ministry of Health, P.O. Box 1234, Woreda 3, Kebele 53, Tel. 152054, Fax 51 26 91, Addis Ababa, Ethiopia and upon payment of a non-refundable fee of birr 100.  The method of payment will be direct deposit to Health Bureau of Benishangul-Gumuz National Regional State, Account No. GOV-42, Commercial Bank of Ethiopia, Assosa Branch.  The document will be sent by surface mail or courier.

4. Bids must be delivered to the address below:

Office of Planning and Programming Department, Ministry of Health, P.O. Box 1234, Woreda 3, Kebele 53, Tel. 15 20 54,k Fax 51 26 91, Addis Ababa, Ethiopia at or before 10:00 A.M. on December 28, 2004.  All bids must be accompanied by a security of not less than 2% of bid amount.  Late bids will be rejected.  Bids will be opened in the presence of bidders/representatives who choose to attend at the address below: Office of Planning and programming Department Meeting Hall, Ministry of Health, Woreda 3, Kebele 53 Addis Ababa, Ethiopia at 10:30 A.M. on December 28, 2004.

Health Bureau reserved the right to reject all or part of the bid.
